Active Home Health, Hospice & Personal Care is looking for a caring, professional, and self-motivated Licensed Practical Nurse (LPN) to join our team. We are a fun-loving, compassionate group that finds purpose in caring for others and supporting patients and their families. 

We offer full-time salaried and hourly opportunities. 

Compensation: 

• Full-time salaried: Starting at $60,000 per year, depending on experience, plus applicable overtime.
• Hourly: $30–$32 per hour, depending on experience, plus applicable overtime.

Job Responsibilities: 

• Provide nursing care to home health and hospice patients under the supervision and direction of an RN, according to provider orders and the established plan of care.
• Monitor patients’ conditions, collect clinical information, and promptly report changes or concerns to the RN Case Manager.
• Administer medications and provide treatments and wound care as ordered, within the LPN scope of practice, demonstrated competency, and agency policies.
• Complete accurate and timely documentation of care provided and patient responses in our electronic health record.
• Assist with implementing the patient’s plan of care and coordinating services in collaboration with the RN Case Manager and interdisciplinary team.
• Provide and reinforce education for patients, families, and caregivers according to the established plan of care and within the LPN scope of practice.
• Collaborate with the home health and hospice interdisciplinary teams to support patient goals and communicate care updates.
• Participate in assigned on-call visits and support duties within the LPN scope of practice, with RN supervision and prompt escalation of changes or concerns.
• Maintain strong communication with patients, families, caregivers, and the care team.

Job Qualifications:

• Current, active LPN license or valid multistate privilege authorizing practice in Utah.
• Caring, compassionate, and professional.
• Strong observation, problem-solving, and verbal and written communication skills.
• Self-motivated with good organization and time-management skills.
• Current CPR certification.
• Valid driver’s license and a reliable vehicle that is insured, registered, and in good working order.
• Comfortable using a smartphone and electronic documentation software.
• Ability to pass required background screening.
