Active Home Health, Hospice & Personal Care is looking for a Full-Time On Call RN for 7 days on & 7 days off Weekday's 5pm - 8am in SLC County.   We are looking for someone who is caring, professional and self-motivated to join our team. Job Requirements:

Minimum 2 years experience in Home Health & Hospice Preferred

7 days on 7 days off weekday's 5pm-8 am

Visit and assess patients to complete an initial comprehensive OASIS or assessment and ongoing assessments through end of care as needed

Answer all phone calls, triage situation, make scheduled and PRN Visits

Chart all clinical documentation in our EHR software

Educate and give instruction to patient, family, and caregivers as appropriate per patient care plan. 

Provides medication education and management per physician order

Works hand-in-hand with the IDT and IDG to ensure patient goals are met and appropriate outcomes are achieved 

Maintains strong communication with patient, family, caregivers, and the interdisciplinary teams

Job Qualifications:

Self-motivated

Ability to pass a background check

Good communication and smartphone skills

Must be a licensed driver with an automobile that is insured, registered and in good working order

Maintains a current CPR certification

Excellent observation, problem solving, verbal and written communications

Polite and compassionate

Requires minimum 2 year Home Health & Hospice experience Preferred

Registered nurse with current license to practice professional nursing in the state of Utah
