Active Home Health, Hospice & Personal Care is looking for a Full-Time, Part-Time & PRN  Physical Therapist.  We are looking for someone who is caring, professional and self-motivated to join our team. Qualifications:

Graduated from a PT curriculum approved by one of the following:

-The American Physical Therapy Association.

-The Committee on Allied Health Education and Accreditation of the Americal Medical Association.

-The Council on Medical Education of the American Medical Association and the American Physical Therapy Association.

 2 years of appropriate experience as a PT preferred

Responsibilities:

• Understands and adheres to established policies and procedures.
• Provides physician and IDT prescribed physical therapy.
• Participates with all other health care personnel in patient IDT care planning.
• Performs all procedures as ordered by physician.
• Consults with physicians and IDT regarding change in treatment.
• Instructs caregiver in use of good body mechanics for turning and lifting patients.
• Instructs patients and family/significant others in home programs and activities of daily living.
• Participates in in-service programs and presents in-service programs as assigned.
• prepares clinical and progress notes accurately and timely per Active's Policy.
• Helps develop the plan of care and revises, as necessary.
• Consults with family and Home Health personnel.

Working Environment:

Works indoors in Home Health office and patient homes and travels to/from patient homes.

Pay is per visit, and commensurate with experience.
